Your Rights under HIPAA

ACE will protect the privacy of your health information (known as Protected Health Information or PHI) for yourself and your family member(s), in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and any applicable state law. PHI generally will be used only for health plan payment activities and operations and in other limited circumstances such as where required for law enforcement and public health activities.

In addition, the minimum necessary information will be used except in limited situations specified by law. Other uses and disclosures of PHI will not occur unless you authorize them. You will have the opportunity to inspect, copy, and amend your PHI as required by HIPAA. You can exercise the rights granted under HIPAA free from any intimidating or retaliatory acts. Please see a HR Representative for an authorization form(s) to exercise those rights.

If you believe that your privacy rights have been violated, you may file a complaint with HR. You may also file a complaint with the Secretary of the Department of Health and Human Services, Hubert H. Humphrey Building, 200 Independence Avenue S.W., Washington, DC 20201. Your complaint must be in writing. You will not be retaliated against for filing a complaint.
