On-The-Job Incidents, Injuries and Illnesses


You must report any work-related accident, injury, or illnesses to your supervisor as soon as it occurs and, in any event, no later than the end of your workday that day. If you incur an on-the-job injury or illness that does not require you to see a physician, you must complete an Accident & Injury Report Form. If you incur an on-the-job injury or illness that does require medical attention, you must complete a Worker’s Compensation Reporting Document at the earliest opportunity. These forms can be obtained from HR.
